• Higher percentages (10–12%) apply for:
  • Medicaid-heavy payer mix
  • High-volume urine drug screens (UDS)
  • Prior auth / complex medical necessity documentation
• Lower percentages (6–8%) for:
  • In-network, clean claim histories
  • Predictable volume with lower denial risk
